Backport the upgrade script from the 1.6.0.0 package, plus the fix from https://bugs.launchpad.net/evergreen/+bug/485143

Thanks to Brandon Uhlmann for reporting the problem in the 1.6.0.0 package.

Also remove the vestigial 1.2.3-1.4-upgrade-db.sql script.
